Job Description:
A company is looking for a Care Navigator to provide telephonic
outreach services and coordinate care for members across various
health plans.
Key Responsibilities
Provides outreach services and coordinates care for various member
populations
Assesses members' needs to guide them towards optimal wellness and
facilitates access to resources
Develops relationships with community resources and monitors member
progress toward health goals
Required Qualifications
Bachelor's degree in social services, behavioral health, or related
field
2 years of experience in care coordination activities, preferably
in a health plan or clinical setting
Experience working with populations with complex biopsychosocial
needs
Certification in Care Coordination or other industry-recognized
certification preferred
Intermediate Microsoft Office skills and knowledge of community
resources
