Job Description:
A company is looking for a Remote Licensed Practical Nurse (LPN)
with California Licensure.
Key Responsibilities
Manage patient census to close gaps in clinical and non-clinical
patient care
Identify and coordinate community resources for patient care
Provide patient education on chronic condition management and
perform medication management
Required Qualifications, Training, and Education
Active California LPN or LVN license
At least 3 years of direct patient-facing work experience
Technical aptitude with Microsoft Office Suite
Experience with chronic care management
Ability to work in a remote, HIPAA-compliant workspace
